- Rehabilitation application form
- Pension insurance number and personal details
- Medical report S0051 from the treating doctor; the relevant G0612 report for children
- Relevant findings, examinations and discharge reports
- Treating doctors and facilities
- Job details and limitations affecting work
- Evidence of sick leave and benefits received
- A reasoned clinic preference if requested

For Deutsche Rentenversicherung Westfalen members, the procedure may include an expert report commissioned by the institution. Steps are not identical for every responsible body.
